​妊娠合并宫颈癌患者的管理

宫颈癌是妊娠女性中最常见的恶性肿瘤之一,随着宫颈癌发病的年轻及女性首次生育年龄的推迟,妊娠合并宫颈癌也成为妇产科医师关注的问题。一般将妊娠期及产后6个月内发现的宫颈癌定义为妊娠合并宫颈癌,又称为妊娠相关性宫颈癌。延迟治疗至胎儿成熟还是立即终止妊娠接受治疗是患者和医生必须共同做出的困难选择。

妊娠期宫颈癌临床表现

妊娠期宫颈癌的临床表现与非妊娠期宫颈癌的临床表现基本相同。常表现为偶发性或性交后阴道流血。由于发生在妊娠期,常易误诊为先兆流产、前置胎盘、胎盘早剥、早产等,可能因为恐惧流产避免阴道检查而导致误诊。

妊娠期宫颈癌筛查

妊娠期宫颈癌的筛查可按照一般诊断程序进行,即细胞学、阴道镜、活组织检查,当怀疑有浸润性病变时可进行诊断性锥切。要特别注意,由于可能导致早产、胎膜早破、感染等发生,妊娠期宫颈活组织检查禁忌做宫颈诊刮检查。

1.如细胞学结果为ASCUS切HR-HPV阳性则需行阴道镜检查;

2.如细胞学结果为LSIL,可以待产后行阴道镜检查;

3.若细胞学结果提示ASC-H、HSIL或AGC-AIS,则需要行阴道镜检查:

1)若阴道镜结果为阴性或CIN I,一年后复查;

2)若阴道镜结果为CIN II,可待产后再治疗;

3)若阴道镜结果为CIN III,可暂不治疗,但需严密检测,每三个月进行一次阴道镜检查以及时发现病情变化;

4)如阴道镜结果提示浸润癌,需立即接受治疗。

妊娠期宫颈癌治疗

目前对于妊娠合并宫颈癌患者的临床管理办法还没有统一定论。为妊娠合并宫颈癌患者制定治疗方案应综合考虑宫颈癌的分期及胎儿的孕周。

国际癌症、不孕与妊娠网络(the International Network on Cancer,Infertility and Pregnancy,INCIP)协会推荐:

1.IA1期患者无淋巴管浸润可行宫颈锥切术;

2.有淋巴管浸润的IA1期、IA2期和IB1期宫颈癌患者:

1)孕周<22周者:行淋巴结分期手术,如淋巴结阳性则建议终止妊娠,如淋巴结阴性可行宫颈切除或待产后治疗;

2)孕周>22周者:新辅助化疗或待产后治疗;

3.IB2期宫颈癌患者:

1)孕周<22周者:可接受新辅助化疗;或行淋巴结分期手术,如淋巴结阳性则建议终止妊娠,如淋巴结阴性可行新辅助化疗或待产后治疗;

2)孕周>22周者:新辅助化疗或待产后治疗;

4.IB3期宫颈癌患者:

1)孕周<22周者:终止妊娠或接受新辅助化疗;

2)孕周>22周者:新辅助化疗或待产后治疗;

5.II期及以上宫颈癌患者:不推荐继续妊娠

李芳主任团队开展临床和临床相关基础研究。

同济大学附属东方医院本部妇科是妇科学博士后和硕博士研究生培养点,医师规范化培训基地。主要研究方向:高危HPV整合相关致癌机制、三代转录组和宫单细胞测序揭示宫颈癌免疫微环境和宫颈癌免疫治疗新靶点。李芳教授以通讯作者身份在Nucleic Acids Research, J Exp Clin Cancer Res.等期刊上发表40余篇学术论文。累计影响因子251.4分,最高影响因子20.69分。主持国家自然基金、科技部重大项目、上海市科委、卫健委和上海市申康课题20余项。主编论著2部。获省部二等奖1项(2/7)。国家级 宫颈病专业精英奖 1项。获批发明专利3项,实用新型专利5项。知识产权转化1项:显微内镜在体病理诊断子宫颈病变。实验室依托同济大学附属东方医院转化医学研究中心,拥有具有完成国家级科研项目所必需的先进仪器设备和充足的科研经费。
